Search for red light therapy and you’ll find it credited with an extraordinary range of effects — pain relief, faster healing, better skin, hair regrowth, fat loss, improved sleep, sharper thinking, slower aging, and a general improvement in more or less everything.
Some of those claims rest on reasonable clinical evidence. Some rest on preliminary research. Some rest on nothing much at all beyond enthusiasm and a sales page.
We use red light therapy in our clinic, and precisely because we do, we think it’s worth being clear about where it stands. Here’s an honest breakdown.
First, the Regulatory Reality
This is dull but genuinely important, because it’s where most misleading marketing originates.
“FDA cleared” and “FDA approved” are not the same thing. Most red light therapy devices reach the market through the FDA’s 510(k) clearance pathway, which requires a manufacturer to demonstrate that their device is substantially equivalent to one already legally on the market. It is not the rigorous premarket approval process that pharmaceuticals go through. Essentially no consumer red light device is “FDA approved,” and any company using that phrase is either confused or hoping you are.
Clearance applies to specific indications only. The cleared uses for these devices are typically narrow — things like temporary relief of minor muscle and joint pain, relaxation of muscle spasm, temporary increase in local blood circulation, and temporary relief of minor stiffness. Separate clearances exist for certain acne and hair growth devices. That’s roughly the scope. A device cleared for temporary muscle pain relief has not been reviewed for anything beyond that.
“FDA registered” means almost nothing. It indicates the manufacturer filed paperwork and paid a fee. It carries no assessment of safety or effectiveness for any claim.
The Federal Trade Commission has taken action against companies in this space for overstating what these devices do, with body contouring, weight loss, and anti-aging claims drawing particular attention. When you see a claim well outside that narrow cleared scope, that’s the context to hold it in.
Where the Evidence Is Reasonably Encouraging
With that framing in place, there is a real research base here — several decades of it — and some areas look genuinely promising.
Musculoskeletal pain conditions. This is the best-supported territory and the reason it appears in physical therapy clinics. Reviews of the clinical literature point to potential benefit for a set of conditions including temporomandibular (jaw) disorders, neck and myofascial pain, tendon disorders, plantar fascia pain, and knee osteoarthritis. A frequently cited meta-analysis published in The Lancet in 2009, pooling randomized placebo-controlled trials in chronic neck pain, found meaningful reductions in pain compared with placebo.
Tendinopathy. Reviews specific to tendon conditions have reported improvements in pain and function when photobiomodulation is used alongside standard care. Note the “alongside” — the loading program remains the primary intervention.
Wound healing. There’s a reasonable evidence base for photobiomodulation supporting wound healing, including in difficult-to-heal wounds, with improvements reported in healing time and associated pain.
Oral mucositis. One of the strongest applications, though outside our scope — photobiomodulation is used in oncology settings to reduce the severity of mouth ulceration caused by cancer treatment, and it appears in some formal clinical guidelines for that purpose.
Certain skin and hair applications. Some devices hold specific clearances for acne and for pattern hair loss, and there’s supporting trial evidence in those areas.
Two honest caveats apply across all of this. First, many trials are small, and study quality varies. Second — and this is the recurring theme — protocols differ so widely between studies that comparing them is difficult. Different wavelengths, doses, and durations aren’t really the same intervention.
Where the Evidence Is Mixed or Thin
Non-specific low back pain. Despite being one of the most common reasons people seek treatment, results here have been inconsistent across studies. Some reviews report benefit; others don’t. This is a case where the honest answer is that it may help some people and we can’t reliably predict which.
Athletic recovery and performance. There’s genuine interest and some supportive preliminary work around muscle soreness and recovery, but this remains an emerging area rather than a settled one.
Skin aging. Some trials report improvements in wrinkles and skin texture. Effects tend to be modest, require sustained use, and the evidence is less robust than the marketing suggests.
Sleep, mood, energy, and cognition. These claims appear constantly in consumer marketing. The research is early — mostly small studies and preliminary work. It’s an interesting area of investigation, not an established use.
Where Claims Outrun the Evidence Entirely
Some things get claimed that the evidence simply doesn’t support in the way it’s presented:
- Fat loss and body contouring. This has been a particular focus of regulatory concern about overclaiming.
- “Detoxification.” Not a defined physiological process that light exposure addresses.
- Treating or curing disease. Red light therapy is not a treatment for any disease, and marketing that presents it that way is outside both the evidence and the regulations.
- Reversing aging. Well outside any cleared indication.
If a provider or product is making these claims, that’s a meaningful signal about the rest of what they’re telling you.
Safety and Who Should Be Cautious
Red light therapy has a good safety profile. It’s non-invasive, doesn’t involve UV, and adverse effects in the literature are uncommon and generally mild — occasional temporary redness, warmth, or mild headache.
That said, some situations warrant caution or a conversation first:
Eye protection. Direct exposure to bright LEDs or lasers can be harmful to the eyes. Appropriate eyewear should be provided and used.
Photosensitizing medications. A number of common drugs increase light sensitivity — some antibiotics, certain diuretics, some acne medications, St. John’s wort, and others. Tell your provider everything you take.
Active cancer or an undiagnosed lesion in the treatment area. Because photobiomodulation influences cellular metabolism, treatment directly over a known or suspected malignancy warrants discussion with your oncology team first.
Pregnancy. Direct treatment over the abdomen is generally avoided.
Photosensitivity conditions. Lupus, porphyria, and similar conditions need individual assessment.
Thyroid. Treatment directly over the thyroid gland is commonly avoided as a precaution.
Recent steroid injections, active infection, or skin conditions in the area — worth mentioning to your provider.
Home Devices Versus Clinic Devices
A common question, and a fair one. The honest answer is that device quality varies enormously.
Clinical systems generally deliver higher power output, allowing shorter sessions and better delivery to deeper tissue. Home devices span a huge range — some are well-built and well-specified, many are marketed with vague or absent specifications and deliver a fraction of the output implied.
If you’re evaluating a home device, look for clearly stated wavelengths, stated irradiance at a stated distance, and a verifiable FDA clearance number. Vague power claims and no wavelength information are red flags.
Most importantly: a device on its own doesn’t tell you what to do with it. Knowing which tissue to target, at what dose, how often, and alongside what else is where the value actually lies.

Red light therapy is a legitimate tool with a real mechanism and a genuine — if uneven — evidence base, particularly for musculoskeletal pain and tissue healing. It’s low risk. For the right person and the right condition, it can be a useful part of a treatment plan.
It is not a cure-all, it is not a substitute for rehabilitation, and the biggest determinant of your outcome will almost always be the loading program, the strength work, and the movement — not the light.
